Na 1 week evaluatie van iv behandeling met arts-microbioloog en/of internist-infectioloog om eventueel te switchen naar oraal regime.
Beta-lactam en vancomycine: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
