Remarks:

Bij hemodynamische instabiliteit (opname IC): voeg vancomycine toe en overweeg toevoegen anidulafungin op geleide inventarisatiekweken. Stop antibacteriële profylaxe met uitzondering van PCP profylaxe.

meropenem iv 2000mg 3dd

General comments

  • Clostridioides difficile moet uitgesloten worden.
  • Behandelduur in overleg met arts-microbioloog en infectieziekten. Over het algemeen ten minste tot aan resolutie neutropenie.
  • Beta-lactam en vancomycine: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
  • Bij vancomycine is spiegelcontrole noodzakelijk
