General comments

  • Fluconazol step down na initiële behandeling met echinocandine: bij klinische verbetering, hemodynamisch gestabiliseerde patiënten, negatieve follow-up bloedkweek, aangetoonde fluconazolgevoeligheid. Niet bij C. glabrata, C. krusei, of bij patiënten met Candida endocarditis, intravasculaire candidiasis, niet-verwijderde centrale veneuze lijnen of prothese-geassocieerde candidiasis.

  • Behandelduur van candidemie bij niet-neutropene patiënten: 2 weken na de laatste positieve bloedkweek mits er geen aanwijzingen zijn voor strooihaarden.

  • Behandelduur van candidemie bij neutropene patiënten: 2 weken na de laatste positieve bloedkweek mits de neutropene fase voorbij is en er geen aanwijzingen zijn voor strooihaarden.

  • Bij acute gedissemineerde candidiasis ten minste 4-8 weken, afhankelijk van resolutie van strooihaarden.
