Priority: 1st choice	Medication: ceftriaxon iv 2000mg 1dd dd 2 to 6 weeks + metronidazol iv 500mg 3dd 2 to 6 weeks	Remarks: Bij sepsis toevoegen ciprofloxacine 400mg 3dd IV. *Alléén* bij patienten met bekende gistkolonisatie, hospital-acquired mediastinitis en/of recent antibioticagebruik: voeg anidulafungin toe in afwachting van kweken. Step-down naar fluconazol conform candidemie pagina bij gekweekte gisten; of stop bij geen gekweekte gisten. Behandelduur afhankelijk van source control en klinisch beloop.

Overweeg drainage
Definitieve antibiotica op geleide van diepe puskweken afgenomen voor start antibiotica
Behandelduur in overleg met arts-microbioloog of infectioloog en afhankelijk van kliniek, drainage en verwekker(s). Over het algemeen 2 - 6 weken.

Beta-lactam antibiotica: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.

Beoordeel gerapporteerde penicilline allergie aan de hand van de anamnese.
